Forms for Attorneys
The Board has forms for attorneys to:
- report compliance on-line;
- request a temporary exemption or extension;
- record credit from teaching an accredited CLE course and certain part-time teaching of law courses; and
- certify MCLE compliance for a request to return to active status.

Application for Temporary Exemption from IL MCLE
Requirements (.doc)  The deadline to submit a request for a temporary exemption has passed for all attorneys whose reporting period ends June 30, 2008. Any application for temporary exemption submitted between now and June 30, 2008 must reference illness, financial hardship, or other extraordinary or extenuating circumstances beyond the control of the attorney that arose on or after June 1, 2008, such as unplanned hospitalization. Beginning July 1, 2008, applications for temporary exemptions for all attorneys whose reporting period ends June 30, 2008 will not be reviewed because the applicable reporting period has ended. Application for Extension of Time to Comply with IL
MCLE Requirements (.doc) 
The deadline to submit a request for an extension has passed for all attorneys whose reporting period ends June 30, 2008. Any application for extension submitted between now and June 30, 2008 must reference illness, financial hardship, or other extraordinary or extenuating circumstances beyond the control of the attorney that arose on or after June 1, 2008, such as unplanned hospitalization. The office of the MCLE Board is paperless. We are committed to accepting and processing applications and related communications electronically. Instructions are on the relevant application. Paper submissions will be destroyed after 30 days and returned only at the applicant's request and cost. The Board's e-mail server has a 5 megabyte per e-mail limit. An e-mail that exceeds 5 megabytes will be returned to the sender automatically by the server. If this happens, please re-submit by zipping the attachments or dividing the attachments into more than one e-mail. If you encounter difficulties with an electronic submission, please contact the office before making the submission. |
